"Your Thyroid Was Tested. It's Normal."

Maybe you've heard that sentence more than once. And maybe, sitting in the parking lot afterward, you ran through the list again anyway: bone-tired by mid-afternoon, no matter how you slept. Brain fog that makes you re-read the same email three times. Hair coming out in the shower, enough that you've stopped looking at the drain. Weight that will not move even though nothing about your eating has changed. Cold hands in a warm room.

You are not imagining any of it. And here is the detail almost nobody mentions at that appointment: "a thyroid test" usually means exactly one number got checked.

Not a panel. Not a picture of your thyroid doing its job. One number.

What "We Checked Your Thyroid" Usually Means

When a doctor says your thyroid was tested, in most cases that means a TSH test. TSH stands for thyroid stimulating hormone. It is the signal your brain sends to your thyroid, a message that says "make more hormone" or "ease off."

Sit with that for a second, because it changes the whole conversation. TSH is not a measurement of your thyroid hormones. It is a measurement of the request your brain is making. It's one message in a longer chain, and everything that matters to how you feel happens further down that chain: the hormone your thyroid actually produces, the conversion of that hormone into its active form, and whether that active form is reaching your cells.

There is a fuller panel that shows the whole chain. Most women are never offered it, not because anyone is hiding it, but because a TSH-first approach is simply the standard way thyroid screening gets ordered.

The One Number That Can Hide the Problem

Here is the core gap. Because TSH is a signal from your pituitary gland rather than a readout of thyroid output, it can sit comfortably in the normal range while the amount of usable thyroid hormone reaching your cells runs low. The chain can break downstream of the signal. When it does, the one number that got checked is the one number that can hide the problem.

There is a second gap worth naming briefly. The "normal range" on a lab report is a statistical range built from a broad population. It is not the level at which you, specifically, feel well. A value can be technically in range and still be wrong for your body. For now, hold onto the simpler point: one number, read against a generic range, is a thin basis for the sentence "your thyroid is fine."

What a Complete Thyroid Panel Actually Looks At

So what does the fuller picture include? A complete thyroid panel adds four kinds of information on top of TSH, and each one answers a question TSH cannot.

The first is Free T4. This is the storage form of thyroid hormone, the reserve your thyroid produces and your body holds ready. Free T4 tells you whether the gland is actually making hormone in response to the brain's request. But T4 itself is not what makes you feel awake and warm and clear-headed, which brings us to the next marker.

Free T3 is the active form, the hormone your cells actually run on. Your body converts T4 into T3, and that conversion step is where things quietly go wrong for a lot of women. Free T3 is the marker most closely tied to how you feel day to day. If you only remember one marker from this article, make it this one.

Then there is Reverse T3, an inactive form your body produces from the same T4. Think of Free T3 as the gas pedal and Reverse T3 as the brake. Under stress, illness, or chronic under-eating, your body shifts conversion toward the brake. You can be making plenty of hormone and still feel slow, because the brake is on while the gas pedal barely gets pressed. Neither pedal shows up on a TSH test.

Finally, a complete panel checks TPO and TG antibodies, two markers that answer a different question entirely: is your immune system attacking the thyroid gland itself? A positive antibody points toward an autoimmune process, and what that means deserves its own conversation. Five markers, one chain: the request, the reserve, the active hormone, the brake, and the immune system's role in all of it. That is what "checking your thyroid" looks like when it's complete.

What We Actually Find in Practice

When women come to our practice with years of symptoms and a folder of "normal" labs, a few patterns show up again and again.

The most common one is a normal TSH sitting on top of a low Free T3. The brain's signal looked fine, so she was told she was fine, while the active hormone her cells depend on was running low the entire time. Nobody lied to her. The test that would have shown it simply was never run.

Another pattern: antibodies that were never checked, then come back positive. For a woman who has spent years being told her fatigue is stress, or age, or something to take up with a therapist, that result can land with surprising force. Not because it's good news, but because it's a name. A driver she had been carrying without a word for it.

And I want to be careful here, because a lab value is not a verdict. A low Free T3 or a positive antibody is a pattern to understand, not a diagnosis handed to you by a blog post. Every result on a panel has to be read in context: your symptoms, your history, your other labs, how you actually feel at 3 p.m. on an ordinary day. A panel is information. Information interpreted against your real life is what changes the picture.
