On Thyroid Medication But Still Feel Terrible? Here's What 'Normal' Labs Miss

You got diagnosed. You started medication. You sat through the dose adjustments, the rechecks, the waiting to see if this was the number that would finally fix things. And after months of tweaking, your doctor tells you the good news: your levels are normal now.
So why do you still feel terrible?
For some women, it's worse than that. The fatigue is heavier than it was before treatment. The weight keeps climbing. The brain fog hasn't lifted. You look at your lab report that says everything is fine, and you want to scream, because your body clearly did not get the memo.

Here's the good news buried in that frustration: a "normal" TSH on medication is the start of feeling well, not the finish line. If you still feel awful, your body isn't being dramatic. It's telling you to look wider.
What "normal" actually means once you're medicated
"Normal" means your result landed somewhere inside the reference range printed on the lab report. That range comes from a big population of people, not from you. It is wide enough that you can sit comfortably inside it and still feel awful.
There's a second problem that shows up specifically after diagnosis. Once you're on medication, a lot of doctors monitor you with a single TSH test at refill time. They never look at Free T3, Reverse T3, or your antibodies again. So the thing they're calling "normal" is often just one number, checked in isolation, year after year.

What matters for you, right now, is that a normal TSH on a refill slip is not proof that your thyroid is well managed.
Your body still has to convert the medication
Levothyroxine, the most commonly prescribed thyroid medication, is T4. T4 is the storage form of thyroid hormone. It isn't the active form your cells actually run on. To use it, your body has to convert T4 into T3.
One of the most useful explanations I've seen comes from patients themselves, who describe Free T3 as the gas pedal and Reverse T3 as the brake. Free T3 is the active hormone that drives your metabolism, your energy, your brain. Reverse T3 is an inactive version that parks itself on the same receptors and blocks them. You can be on a perfect dose of levothyroxine, have a textbook TSH, and still convert poorly, so your gas tank reads full while your foot can't reach the pedal.
What pushes conversion in the wrong direction? Chronic stress and high cortisol are big ones. So is inflammation, low iron, and being short on the nutrients your body needs to do the conversion at all. Crash dieting does it too.
This is why the fix isn't always "more medication." If your T4 is fine but it isn't becoming usable T3, raising the dose just gives you more raw material you still can't convert. The question to bring to your doctor is whether your Free T3 and Reverse T3 have ever actually been measured, not just your TSH.
The Armour question, and why it might have felt different
A lot of women have the same story: they felt good for years on one medication, then got switched to another and slowly fell apart. Often the one that worked was Armour or another natural desiccated thyroid, and the switch was to levothyroxine.
That isn't a coincidence, and it isn't nostalgia. Levothyroxine is T4 only. Natural desiccated thyroid and combination prescriptions contain some T3 as well, which means they hand your body a little active hormone directly instead of relying entirely on conversion. If you're someone who converts poorly, that direct T3 can be the difference between functioning and dragging.
I want to be careful here, because this is the part of the conversation that gets people into trouble. This is a discussion to have with the person who prescribes your medication. It is not a cue to change your dose, switch formulations, or stop anything on your own. Adjusting thyroid medication without supervision can be genuinely dangerous. What you can do is notice the pattern, write down when you felt your best and what you were taking, and bring that history to your next appointment as a real piece of evidence.
Are you actually absorbing your dose?
This is the least glamorous item on the list and one of the highest payoff, so don't skip it.
Thyroid medication is fussy about how you take it. Food, coffee, and certain supplements can blunt how much actually makes it into your blood, which means the dose on the bottle is not the dose you're getting.

Getting the most from your thyroid medication
- Take it on an empty stomach, first thing in the morning.
- Wait 30 to 60 minutes before coffee or food.
- Keep it several hours apart from calcium and iron supplements.
- Keep it apart from magnesium and antacids too.
- If you take a proton pump inhibitor or other acid-reducing medication, mention it to your doctor, because low stomach acid affects absorption.
There's a deeper layer here as well. Gut problems like low stomach acid, celiac disease, or SIBO quietly reduce how much you absorb. So if your gut isn't well, your perfectly calculated dose can still come up short. When it's not your thyroid: the deficiencies that imitate it
Here's the reframe that changes everything for the women I work with.
Fatigue, brain fog, hair loss, and low mood are not thyroid-specific symptoms. They are what your body does when something is off, and a long list of common deficiencies produce the exact same picture. So when your thyroid is treated and you still have those symptoms, the answer might not live in your thyroid at all.
These are the usual suspects, and each one is worth testing:
Worth checking when a treated thyroid still leaves you exhausted
- Ferritin and iron. Low iron alone causes fatigue, hair loss, and breathlessness, and your body needs iron to use thyroid hormone properly.
- B12 and folate. Low levels bring fatigue, brain fog, and low mood, and they're common if your gut or your diet isn't cooperating.
- Vitamin D. Low D drives fatigue and aches and influences your immune system.
- Selenium, zinc, and magnesium. Your body needs these to convert T4 into T3 and to make energy at all.
A woman who is medicated, "normal," and still wiped out deserves a proper look at what else is low. Not a shrug, and not another year of TSH-only refills.
The things a treated thyroid was never going to fix
Sometimes the thyroid genuinely is handled, and the reason you feel bad is that something else moved in next to it. Treating one condition doesn't make the others disappear.
Perimenopause is the big one, and it overlaps with thyroid symptoms so closely that the two get confused constantly. Fatigue, weight changes, mood swings, poor sleep, brain fog. If you're in your late thirties or forties and the timing fits, it's worth asking whether some of what you're feeling is hormonal change rather than thyroid failure. Then there's food. Plenty of women with Hashimoto's find that cutting gluten makes a real difference, sometimes a dramatic one, because of how it feeds inflammation in a sensitized body. It doesn't help everyone, but it's worth an honest trial.

How to take this back to your doctor without starting over
You're in a better position than the woman who hasn't been diagnosed yet. You don't have to fight to be believed. You just need to widen the investigation, and you can do that with one clear request.
Here's something you can bring with you:
"I'm on thyroid medication and my TSH is in range, but my symptoms haven't improved. I'd like to check Free T3, Reverse T3, ferritin, B12, folate, vitamin D, zinc, and selenium. I'd also like to talk about whether a T4 and T3 combination is worth trying."

The bottom line
Treated is not the same as optimized, and it's definitely not the same as done.
A normal TSH on medication means your thyroid is being managed. It does not mean every reason you feel terrible has been found. If you're still exhausted, still foggy, still not yourself, the data isn't telling you to give up and accept it. It's telling you that the next answer is somewhere you haven't looked yet.
You deserve to feel like yourself again. Often that comes down to checking the things a standard refill appointment never gets to.
